
The Gauteng Department of Education has launched a full-scale independent investigation into the heartbreaking death of 10-year-old Milton Mokgoatsane, a Grade 5 learner who was fatally injured when a goalpost collapsed on him at Reagile Primary School in Tembisa on 16 February.
What began as an ordinary school day ended in unimaginable tragedy for Milton’s family, his classmates, and the school community. Paramedics who responded to the incident declared the child dead at the scene, leaving behind a wave of grief and calls for accountability.
Independent Law Firm to Lead Investigation
Gauteng Education MEC Matome Chiloane has now appointed a law firm to investigate the circumstances that led to Milton’s death. According to department spokesperson Steve Mabona, the MEC will formally introduce the law firm to the school on Wednesday, where he is expected to outline the terms of reference that will guide the investigation.
The independent probe will focus on:
- The condition and stability of the goalpost
- Safety protocols and supervision at the time of the incident
- Whether the school adhered to infrastructure safety standards
- Possible negligence or oversight
- Recommendations to prevent future tragedies
The move comes amid growing public concern about school safety and infrastructure management across the province.
Heartbreak and Unanswered Questions for Family
Family spokesperson Thomas Ndobe expressed the family’s devastation following Milton’s sudden and preventable death. Ndobe said the family is awaiting a detailed report from the independent investigators, hoping it will bring clarity and closure.
“For a child to leave home healthy and never return, it’s a pain no family should ever experience,” Ndobe said earlier. “We want answers. We want to understand how such a tragedy could happen at school.”
Another Tragedy Earlier in the Month
Milton’s death is not an isolated incident. Just two weeks earlier, on 2 February, five-year-old Manqoba Mnisi, a Grade R learner at Bernard Isaacs Primary School in Coronationville, sustained injuries under mysterious circumstances and later died in hospital.
Manqoba’s mother has raised disturbing concerns, alleging that her son may have been locked inside a classroom before becoming trapped in a window while trying to escape. The Gauteng Department of Education is also probing that case.
A Concerning Pattern Across Schools
Adding to the growing alarm, the department confirmed that it is investigating another recent case in which a learner died inside a classroom under yet-to-be-explained circumstances.
Beyond Gauteng, a troubling incident in the Eastern Cape has further shaken the education sector. A Grade 3 learner died by suicide reportedly after enduring bullying highlighting the widespread and deeply entrenched challenges facing schools nationwide.
Parliament Calls for Action Against Bullying
Reacting to the Eastern Cape case, Parliament’s Education Portfolio Committee chairperson Makhi Feni expressed shock and called for a thorough investigation.
“It is not normal that a learner so young would commit such an act,” Feni said.
“We are calling for a full investigation into this incident. Bullying is a challenge in our schools, and educators should never be tired of fighting it.”
The committee emphasized that bullying, inadequate supervision, and failing infrastructure remain urgent issues requiring decisive intervention.
Growing Demand for Safer School Environments
The recent string of tragedies has intensified public scrutiny over safety standards in South Africa’s schools. Parents, teacher unions, and civil society groups are increasingly voicing frustration over recurring infrastructure failures, overcrowded classrooms, and inadequate supervision during break times and sports activities.
